New Patients We ask all new patients to complete the following Medical Questionnaire and forward it to the surgery prior to your first appointment. Many patients prefer to download the form then fill it in off line and bring it with them when they make their first visit. Here are two versions for you to download and print off. DID YOU KNOW If you are 10 mins late we will more often than not rebook the appointment. Failure to make an appointment: If you fail to attend you will be asked to make a deposit. Two failed appointments in 2 years will result in you being removed form our patients list.
